Milan rely on the same XI that won the first leg of this Champions League quarter-final with Rafael Leao and Olivier Giroud, while Napoli bring in Victor Osimhen with Kvicha Kvaratskhelia.
It kicks off at the Stadio Diego Armando Maradona at 20.00 UK time (19.00 GMT).
The first leg ended 1-0 for the Rossoneri at San Siro with a goal from Ismael Bennacer, while Andre Frank Zambo Anguissa was sent off for two bookable offences.
Because of that, both Anguissa and defender Kim Min-jae are suspended, with Giovanni Simeone injured, so Tanguy Ndombele and Juan Jesus step in.
Another big change is the return of Osimhen, who was out with a muscular problem in both that game and the 4-0 Serie A defeat to Milan on April 2.
Milan stick to the same XI that won the first leg last week, with Rafael Leao and Brahim Diaz supporting Giroud, as the France international has shaken off a bruised ankle.
There are 22 points separating these teams in the Serie A table, but the Champions League all comes down to this match.
